CHILDREN'S MINISTRY VOLUNTEER APPLICATION

Working with children is one of the most rewarding parts of serving the Lord in His church. He finds them precious and wants His church to nurture them in the truths of His Word and the wonder of His love.

 

This application is meant to protect you as well as the precious littles ones over whom God has placed us. This application must be completed by all applications (volunteers and employees of Warrior Creek Baptist Church) for any potion involving supervision or custody of minors. This application is used by Warrior Creek Baptist Church to help promote a safe environment for the children that participate in our programs or use our facilities.

 

Any applicant who is a survivor of childhood sexual or physical abuse needs the love and acceptance of Warrior Creek Baptist Church.


Applicants who have such a history should discuss their desire to work with children or youth with the leadership of the church prior to any participation as a children or youth worker.

 

All applicants must be members, study and agree to obey the guidelines that are provided for their program and position within the church’s children/youth ministry.

In order to provide a safe and secure environment for our clients, staff and volunteers here in our children’s or youth ministries, we believe it is necessary to include the following questions as part of our application process. All information will be kept confidential (police may access this information, under warrant, if requested) Answering yes to any of these questions may not necessarily prevent your involvement in ministry.

I understand that if my character or morals are deemed by Warrior Creek Baptist Church’s leadership to be inappropriate and/or criminal at any time during my volunteering, Warrior Creek Baptist Church will be entitled to terminate my volunteering or employment without express cause or prior notice regardless of any other oral or written statement by Warrior Creek Baptist Church.

 

I understand that Warrior Creek Baptist Church is responsible for the welfare of any person or persons entrusted to my care. I will cooperate fully with the director, staff, or other volunteers in the fulfillment of my duties and will keep all information I encounter, in my role as a volunteer or employee, confidential. If at any time I find that for any reason I am unable to support the policies, procedures, or doctrine of Warrior Creek Baptist Church, I will gracefully and quietly resign from my position with the church. If my supervisors find that I am in conflict with any of the policies, procedures or doctrines and we are not able to resolve the issue, I will gracefully and quietly agree to resign from my position with the organization.

These reports may be obtained at any time after receipt of your authorization and, if you are hired, throughout your employment. You have the right, upon written request made within a reasonable time after receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report. Please be advised that the nature and scope of the most common form of investigative consumer report obtained with regard to applicants for employment is an investigation into your education and/or employment history conducted by Clear Investigative Advantage LLC, 11330 Legacy Dr, Suite 307, Frisco, TX 75033, Tel: 888-242-2503 or another outside organization. The scope of this notice and authorization is all-encompassing, however, allowing Employer to obtain from any outside organization all manner of consumer reports and investigative consumer reports now and, if you are hired, throughout the course of your employment to the extent permitted by law. As a result, you should carefully consider whether to exercise your right to request disclosure of the nature and scope of any investigative consumer report.


ACKNOWLEDGMENT AND AUTHORIZATION


I acknowledge receipt of the NOTICE REGARDING BACKGROUND INVESTIGATION and A SUMMARY OF YOUR RIGHTS UNDER THE FAIR CREDIT REPORTING ACT and certify that I have read and understand both of those documents. I hereby authorize the obtaining of “consumer reports” and/or “investigative consumer reports” at any time after receipt of this authorization and, if I am hired, throughout my employment. To this end, I hereby authorize, without reservation, any law enforcement agency, administrator, state or federal agency, institution, school or university (public or private), information service bureau, employer, or insurance company to furnish any and all background information requested by Clear Investigative Advantage or another outside organization acting on behalf of Employer or Organization, and/or Employer or Organization itself. I agree that a facsimile (“fax”) or photographic copy of this Authorization shall be as valid as the original.
